What Medicare Won't Cover in 2026: What You Need to Know

As healthcare costs keep going up, many Americans on Medicare are wondering what changes are coming in 2026.

While Medicare still helps millions of seniors and people with disabilities pay for healthcare, there are some important services it doesn't cover.

This means many people will need to pay for these services themselves.

Dental Care

One of the biggest gaps in Medicare is dental care. Regular dental services like cleanings, fillings, tooth removal, and dentures are not covered.

Some dental work may be covered if it's directly related to another medical treatment Medicare pays for, but most everyday dental needs are not included. This is especially hard for seniors living on a fixed income.

Vision Care

Medicare also doesn't cover most vision services. Routine eye exams, glasses, and contact lenses are usually not included.

However, if you have a medical condition like cataracts or glaucoma, Medicare may help pay for treatment. For basic vision care, you'll likely need separate insurance or pay out of your own pocket.

Hearing Services

Hearing aids and most hearing exams are not covered by Medicare, even though many older adults experience hearing loss.

This can be a big expense since quality hearing aids can cost thousands of dollars.

Long-Term Care

One of the biggest financial concerns for seniors is long-term care. Medicare does not pay for extended stays in nursing homes or assisted living facilities when you need help with daily activities like bathing or eating.

It only covers short-term care in a skilled nursing facility or rehabilitation under certain conditions. Many families have to use their savings or look for other programs to help pay for long-term care.

Why These Gaps Exist

Medicare was created in 1965 to help with serious medical problems, not ongoing care like dental or vision services.

Over the years, lawmakers have talked about adding more benefits, but the high costs have made it difficult to make big changes.

Other Services Not Covered

There are other services Medicare won't pay for:

  • Cosmetic surgery – Unless it's medically necessary
  • Alternative treatments – Things like acupuncture are only covered in very limited cases
  • Some prescription drugs – While Medicare Part D helps with medications, not all drugs are covered, and some may have high costs

What Is Still Covered

The good news is that Medicare still covers many preventive services. This includes health screenings, vaccines, and yearly wellness visits. These services help catch health problems early.

How to Fill the Gaps

To get coverage for services Medicare doesn't include, many people sign up for additional insurance plans like:

  • Medicare Advantage – These plans often include dental, vision, and hearing benefits
  • Medigap policies – These help cover costs that regular Medicare doesn't pay for

Keep in mind that these plans have their own costs and may limit which doctors you can see.

The Bottom Line

Understanding what Medicare covers—and what it doesn't—is important for planning your healthcare costs in 2026.

Make sure to review your coverage options each year and budget for any services you may need to pay for yourself.
